Thank you for agreeing to participate in the Roche Diabetes Care, Inc. (“RDCI”) online focus group research session (“Focus Session”).

RDCI appreciates and values your time and opinions. RDCI will utilize Stratos Innovation Group, LLC (“Stratos”) to perform the Focus Session. Stratos is legally bound to adhere to all privacy laws and regulations, as well as the Code of Standards and Ethics for Survey Research of the trade association, Council of American Survey Research Organizations, (“CASRO”) (www.casro.org).

The main objective of the Focus Session is to gather feedback or fresh ideas regarding insulin pump pre-purchase, onboarding, and post-purchase processes or gather insights related to peer support, brand loyalty, and advocacy.

The Focus Session will last approximately 45 minutes in duration. No one will try to sell you anything. Upon completion of the session, you will receive a $40 smart speaker and a $50 gift card in exchange for your feedback. in the Focus Session. RDCI is obligated to report any transfer of value as required by federal or state disclosure laws, as applicable.

Please review and agree to the following prior to participating in the Focus Session:

1. I understand that the views that I express during the Focus Session will not be attributed to me by name or individual identity.
2. I have been advised that I have the right to refuse to answer any questions that I have privacy concerns about; I also agree to cooperate with the moderator(s) and answer questions related to the Focus Session topic.
3. I agree that I will not disclose information viewed or discussed in the Focus Session and will keep all such information confidential.
4. I understand that I may be shown a prototype of services that are currently under development and have not yet been released or marketed in the United States. I acknowledge that any information I receive or view in this Focus Session about such unreleased services is subject to change.
5. I understand that the Focus Session I am participating in may be video or audio recorded, and I hereby give my permission and release for this recording with the understanding that the recording will be used for internalanalysis for this Focus Session only.
6. I understand that any data shared with me should not be interpreted as containing fully documented claims or features, but potential claims and features that RDCI wants to solicit feedback.
7. I hereby consent and release all and any feedback including but not limited to input, ideas or concepts, I provide, directly or indirectly, during the Focus Session.
8. I acknowledge and agree that the compensation to be provided is based solely on the fair market value for the services provided hereunder and all amounts paid hereunder will be reported to the IRS and other government authorities as may be required by applicable laws.
9. I understand that I have the right to revoke this consent in writing at any time by sending the written revocation to [email protected]. I understand that a revocation is not effective to the extent that Stratos has already acted in reliance on my consent.
10. I understand that by selecting the “I Agree” button below, I am signing this Agreement electronically. I agree my electronic signature is the legal equivalent of my manual signature on this Agreement. By selecting “I Agree” I consent to be legally bound by this Agreement’s terms and conditions. I agree that my use of a key pad, mouse, or other device to select an item, button, icon, or similar act/action, or to otherwise provide Stratos or in accessing or making any transaction regarding any agreement, acknowledgement, consent
    terms, disclosures, or conditions constitutes my signature (hereafter referred to as “E-Signature”), acceptance and agreement as if actually signed by me in writing. I also agree that no certification authority or other third party verification is necessary to validate my E-Signature and that the lack of such certification or third party verification will not in any way affect the enforceability of my E-Signature or any resulting contract between me and Stratos. I further agree that each use of my E-Signature constitutes my agreement to be bound by the terms and conditions of this Agreement.
