CWD Conference Survey

Survey closed April 21, 2002

Dear CWD Families:

Thank you for your tremendous support of the CWD summer conferences over the last 3 years. They have been successful beyond what we had imagined, and this is all because of you.

We have had numerous requests to add different kinds of conferences and different times of the year to our schedule, and we are considering how to expand in the conference arena. There are many possibilities: smaller, bigger, more international, more "vacation-focused," less expensive, more expensive. We need your input to help us define the direction to go.

Please take a few minutes of your time to complete the following survey. All responses are confidential. If you don't want to answer a question, skip it. Please note that in any case, we will be continuing our summer conferences on a west coast/east coast schedule at least through the next several years. Thank you!

  1. Would your family be interested in participating in a recreational conference during the winter holiday break? This gathering would be held someplace warm, with lots of group outdoor activities (golf, tennis, swimming), and would include 1-2 diabetes-related sessions daily, as well as social events.

    Very interested
    Somewhat interested
Not interested at all (please skip to #5)

  2. If so, which dates would you prefer?

Dates between Christmas and New Year's, but not including any of the actual holiday dates
    Dates including New Year
Dates directly after New Year (for example, this year, Jan. 2-5 is a Thursday - Sunday)

  3. Because we would be considering resort areas at high season, the room rates may be higher than those for our summer conference. What would your family be comfortable spending on a room?

    Maximum of $150 per night
    Maximum of $200 per night
    Maximum of $250 per night
    More than $250 per night

  4. Would you be interested in any of the following options?

    A cruise
A resort out of the country (i.e., the Caribbean)

  5. If CWD were to offer smaller, regional, educational conferences at other times during the year (i.e., a Friday evening - Sunday conference), would your family be interested in participating? Please indicate the region of most interest to you:

    Northeast corridor
    Atlantic corridor
    Pacific northwest
    Rocky Mountain corridor
    Not interested

  6. If you participated in one of these regional conferences, how much would you feel comfortable spending on a hotel room?

    Maximum $75 per night
    Maximum $100 per night
    Maximum $150 per night
    Maximum $200 per night

  7. If CWD were to sponsor an international conference in Canada in the autumn months over a long weekend, would you be interested in participating? Which city would most interest you?

    Quebec City
    Not interested

  8. If you have any other comments, please let us know:
